Mr. O’Connor is a 52 year old man who has had type 2 diabetes for 7 years. Mr. O’Connor has calf claudication. He has a blood pressure of 120/75 mmHg.

What is the best approach for vascular protection for Mr. O’Connor?

Mr. O’Connor has end organ damage with macrovascular disease. Diabetes Canada guidelines recommend therapy with:

  • Statin to target LDL <2.0 mmol/L
  • ACEi or ARB in doses that have been shown to have vascular protection
  • ASA (clopidogrel if intolerant to ASA)

Additional learning point: Use ACEi or ARB for vascular protection even in patients with normal blood pressure
